We all know everything in history didn't always work perfectly, and we are here to find out which one's didn't work the most.

There are two categories to a failure:
    1) Something in history (be it a military campaign, an invention, ect.) was working very well, but something went wrong, or someone made a mistake to mess it up. I refer to this as the "Agh! So Close" failure

    2) The second, and perhaps most entertaining to watch would be a failure from the start. A complete and utter goof up that leaves heads spinning and historians wondering why they thought it would ever work. I call this the "Oh God what have I done" failure.

And every attempted US assassination of Fidel. I think we tried somewhere around a dozen different times and screwed it up every time. The US claims "credit" for at least six attempts, Castro claimed it was more like two dozen attempts, I imagine the truth lies somewhere between. Regardless, it was total fail on the CIA's part. I'm glad they failed though. Assassinating foreign rulers due to differences in ideology is very not cool.

Poland-Lithuania's implementation of a national constitution in 1791, thus becoming the second country after the USA to do so.

It ingnited a mass invasion by Prussia, Russia and Austria-Hungary, leading to the complete partition of the country between the three.

That depends on how you look at things. A few Italian city states had constitutions and all of the colonies had their own constitutions before they formed the us. There were also a bunch of countries throughout the middle ages to draft legal documents governing their nations (which, while not using the word constitution, is kind of the same thing) so I wouldn't even call the US constitution the first of its kind. Interesting fact about Poland though, I had no idea.

Sorry, I should have said the world's second codified constitution; i.e. one contained within a single document.
