In hero mode, you take twice as much damage and no hearts appear under normal circumstances (there is a way later on to get hearts to drop, though). To heal up, you need potions, access to a chair to sit and heal, or a bed to sleep in. To counterbalance this a bit, the skyward strike starts out just as powerful as it was at the end of the first playthrough and the charge rate goes down throughout the HM playthrough to nearly instant charge at the end. Further, NPCs recognize that you are playing in Hero mode and let you skip some tutorials.

Edit: Oh, and you keep all of your treasures and bugs from the previous playthrough.
